The Care Coordinator is responsible for coordination of all care needs of assigned Amida Care members, including but not limited to medical specialty, service referrals, Behavioral Health services, hospitalization and home care. Communicates and consults with all members of the Care Management Team and other Primary Care site staff to ensure coordination of care, comprehensive care planning and appropriate interpretation of Amida Care policies to ensure the needs of the assigned members are met.
Qualifications:
Excellent interpersonal communication skills, strong work ethic, energetic, punctual, reliable, strong organizational, analytical and interpersonal skills with detail-oriented focus. Knowledge of Article 49- Utilization Review process. A minimum of two years of prior health care or managed care experience; as well as of HIV/AIDS, medical or chronic care experience. Demonstrated computer proficiency, including but not limited to, Microsoft Office (Word, Excel, Access). Bilingual, Spanish preferred.
Education:
LPN or RN with an Associates or BSN
