You will still have Medicaid -- and all the services you need. Think of it like this: Amida Care becomes your insurance company under Medicaid Managed Care, and we pay for most of your medical and mental healthcare, including doctor visits, hospital care and medications. Meanwhile, regular Medicaid pays for your dental care, COBRA case management, and outpatient substance abuse treatment such as Methadone Maintenance and Day Treatment – if you need it. |

As an HIV+ Amida Care Member, you are guaranteed a Primary Care Provider who is a credentialed HIV specialist. Also, any specialist you see will send all results of their findings directly to your Primary Care Provider. Just choose your Primary Care Provider from the Amida Care Provider network. |

You can -- if your doctor is a participating Primary Care Provider with the Amida Care Plan. It's easy enough to find out if your doctor is part of the Amida Care network. Just call us -- any time, at 1-800-556-0689, and we'll tell you right away. |

Amida Care is a health Plan - with special care coordination services geared toward the needs of people living with HIV/AIDS, and their children. While we're not a housing program like HASA, we still help Members by referring them to housing and supportive services organizations. |

Amida Care is a Special Needs Plan (SNP). There are no limits on the care Members can receive as long as it is medically necessary. At Amida Care we believe that regular ongoing care and HIV monitoring are important to maintaining health. We want our Members to get all the care they need. |

Forget about co-payments. There are none! As an Amida Care Member, you'll make no co-payments whatsoever for any Amida Care covered services. We'll pay those doctors and hospitals directly. You pay nothing to them. |

There's no problem. We make every effort to ensure your complete satisfaction at all times, so if the Plan is not meeting your needs, call our Member Services department. It's important that you let us know if you’re dissatisfied in any way, so we can try to resolve any issues. Of course, if you still want to leave the Plan; simply call NY Medicaid CHOICE at 1-800-505-5678. They'll ask you to choose another Plan. In the meantime, you will likely be dis-enrolled from Amida Care on the first day of the very next month. |

Only adults with HIV/AIDS can join Amida Care. However, Members who are parents or guardians can also enroll their children – whether they have HIV/AIDS or not. |
