1. Search by Name:

Important: Enter First name or Last name ONLY.
A short list will automatically pop-up. If the Provider appears, click their name. For an expanded list click the SEARCH button.

2. Search by Category and Sub-category:

First choose a Category (Primary Care or Specialists), and then choose a Sub-category (medical services).

Pharmacy Network Search

Beacon Health Strategies Behavioral Health Provider Search

To find a Beacon Health provider please go to www.beaconhealthstrategies.com and then click on the “Providers” link on the top of the page. Then click on the “Tools” subheading. This will take you to a login screen where you should enter “Amida Care” and hit enter, which will then direct you to the provider tools page. Amida Care search tool may also provide additional participating behavioral health providers.

Amida Care is a New York State sponsored HIV Special Needs Plan (SNP) that provides health care services for people on Medicaid living with HIV and their children. Amida Care services Manhattan, Brooklyn, the Bronx, Queens and Staten Island. Starting September 1, 2010, if you live in New York City, choosing a Special Needs Plan or a regular Medicaid managed care plan is mandatory for PLWHA.
