style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\"> Your team leader coordinates with your primary care provider, specialists, hospitals, nursing homes, and other resources to ensure you receive appropriate care and services.<\/span><\/span><\/li>\n<li><strong><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\">Case Management Coordinator &#8211;\u00a0<\/span><\/span><\/strong><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\"> Conducts phone assessments with you at least twice a year and helps connect you to community resources such as case management, mental health and substance abuse treatment, housing and legal services, education and transportation assistance, and general health Care wants to help our members living with HIV Live <\/span><em><span style=\"vertical-align: inherit;\">Your <\/span><\/em><\/span><em><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\">Life&#8230;Undetectable<\/span><\/span><\/em><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\"> ( <\/span><span style=\"vertical-align: inherit;\">LYLU). <\/span><span style=\"vertical-align: inherit;\">For most people living with HIV, taking antiretroviral (ARV) medications exactly as prescribed by their Primary Care Provider (PCP) will reduce the amount of HIV in the blood to a very low level. <\/span><span style=\"vertical-align: inherit;\">This is sometimes referred to as \u00abundetectable\u00bb viral load or \u00abviral load suppression.\u00bb <\/span><span style=\"vertical-align: inherit;\">People living with HIV who achieve undetectable viral loads live longer, are healthier, and cannot transmit HIV sexually to others.<\/span><\/span><\/p>\n<p><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\">The <\/span><\/span><a href=\"https:\/\/www.amidacareny.org\/es\/para-miembros\/programa-viva-su-vida-indetectable\/\"><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\">LYLU Program helps<\/span><\/span><\/a><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\"> you take your antiretroviral medications regularly and stay undetectable. <\/span><span style=\"vertical-align: inherit;\">If you enroll in the program and your viral load is no longer detectable, or becomes undetectable, you <\/span><\/span><strong><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\">will receive $100 for each quarter (three-month period) that you remain undetectable<\/span><\/span><\/strong><span style=\"vertical-align: care centers. <\/span><span style=\"vertical-align: inherit;\">These representatives attend quarterly meetings where they discuss their perceptions and evaluations of the Amida Care member experience. <\/span><span style=\"vertical-align: inherit;\">Information and feedback from the MAC helps Amida Care improve its member services. <\/span><span style=\"vertical-align: inherit;\">The MAC also class=\"tab-content\"><\/span><\/span><\/p>\n<p><strong><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\">The Amida Care Workforce Initiative Network (WIN)<\/span><\/span><\/strong><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\"> (also called the <\/span><\/span><em><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\">Peer Training Initiative<\/span><\/span><\/em><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\"> ) is a free vocational training program for Amida Care members that helps them develop marketable employment skills, to enter or return to the workforce. <\/span><span style=\"vertical-align: inherit;\">WIN program participants are trained to use their life experience in jobs related to \u201cnavigating\u201d the health system and services, and outreach. <\/span><span style=\"vertical-align: inherit;\">The program is offered four times a year in collaboration with <\/span><\/span><em><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\">Housing Works<\/span><\/span><\/em><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\"> (six-week training) and ASCNYC (12-14 weeks).<\/span><\/span><\/p>\n<p><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\"><\/div><\/span><\/span><\/p>\n<p><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\"><a class=\"tab-label\" href=\"#transgender-services\">Transgender Services<\/a><div id=\"transgender-services\" class=\"tab-content\"><\/span><\/span><\/p>\n<p><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\">Starting in November 2017, Amida Care&#8217;s Medicaid Live Life Plus Plan is open <\/span><\/span><strong><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\">to all eligible transgender people<\/span><\/span><\/strong><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\"> , regardless of HIV status. <\/span><span style=\"vertical-align: inherit;\">Seven percent of Amida Care&#8217;s members, more than 400 in total, are people of transgender background. <\/span><span style=\"vertical-align: inherit;\">Transgender members have access to a full range of free or low-cost health benefits and services, including:<\/span><\/span><\/p>\n<ul>\n<li><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\">Experienced doctors, clinics, hospitals and pharmacies throughout New York City<\/span><\/span><\/li>\n<li><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\">Specialized transgender health care including hormonal therapies, PrEP and PEP, and gender-affirming surgeries<\/span><\/span><\/li>\n<li><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\">Help with housing referrals, job training and other assistance programs<\/span><\/span><\/li>\n<li><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\">Treatment for mental health conditions, HIV, sexually transmitted infections, and substance use<\/span><\/span><\/li>\n<\/ul>\n<p><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\"><\/div><\/span><\/span><\/p>\n<p><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\"><a class=\"tab-label\" href=\"#home-program\">HOME Program<\/a><div id=\"home-program\" class=\"tab-content\"><\/span><\/span><\/p>\n<p><strong><span style=\"vertical-align: inherit;\"><span style=\"vertical-align: inherit;\">Amida Care&#8217;s Homeless Management, Outreach and Empowerment (HOME) <\/span><\/span><\/strong><span style=\"vertical-align: inherit;\"><strong><span style=\"vertical-align: inherit;\">Program<\/span><\/strong><span style=\"vertical-align: inherit;\"> helps our members who are homeless or at risk of becoming homeless, connecting them to vital social and including connections and referrals to housing, education, legal and employment
