Amida Care members can receive the following services from an in-network vision provider at no cost:
Members with diabetes may receive an eye exam every 12 months.

Amida Care members can receive routine dental care from an in-network dentist at no cost. Covered services include:
Some services, including root canals, crowns, dentures and implants, must meet specific coverage requirements and may require prior approval from Amida Care. Your dentist will submit the request for approval before providing the service.
Dental benefits are provided through Healthplex. You may receive a dental identification card with an assigned dentist; however, you may visit any dentist in the Healthplex network.
