What Is Personal Information?

What Personal Information Is Collected?
While it is possible to use our website without revealing any personal information, we provide a landing page that allows you to send us messages that may contain personal information. The landing page includes an input form in which you can enter your first and last name, your postal address (including your ZIP code), your telephone and fax numbers, and your e-mail address. The input form asks you to select the reason for your message from a drop-down box and allows you to enter text. Personal information is obtained by Amida Care only when voluntarily submitted through this form by you, the visitor.
We also provide links on our website to our e-mail addresses, which are used to facilitate communication between you and Amida Care. You may voluntarily submit information to us through electronic mail. Unless otherwise noted, providing information through the input form on our website and/or through electronic mail is not a secure and confidential means of communication. Non-encrypted electronic communications may be accessed and viewed by other Internet users without your knowledge and permission while in transit to us.
How Is Personal Information Used?
Amida Care does not sell, rent, or trade consumer mailing lists. We use the information collected through our website to respond to your inquiries, for quality and service improvement purposes, to supply you with information about enrollment, events and news at Amida Care, to request donations, and to seek volunteers to contribute time to Amida Care.
What If You Do Not Want Personal Information To Be Collected?
Personal information will not be collected from you without your knowledge and approval. Providing Amida Care with your personal information is entirely optional. You do not have to provide us with personal information if you do not want to, though not providing information may limit your ability to request and receive information from us.
Amida Care also allows users to opt out of receiving e-mail and/or postal mail from us. To opt out of e-mail and/or postal mail, please send an e-mail to HIPAA@amidacareny.org requesting removal from our e-mail and/or mailing list.
How Is Personal Information Protected?
Amida Care protects personal information by using appropriate safeguards, including standard protocols to protect the website files and databases. We will not use or share personal information with any third party, except when permitted or required by federal and/or state law, and, except as necessary to operate the website, we may provide our website hosting service provider(s) with access to information collected through the site.

From time to time we will provide links to other websites not owned or controlled by Amida Care. We do this because we think these outside resources might be of interest or use to you.
