• Amida Care Live Life Advantage enrollment is voluntary. You can withdraw your referral or application at any time during this process up to the point that your enrollment becomes effective.

When Care Begins

Amida Care Live Life Advantage will confirm your Medicare eligibility. In most cases, you will become a member of Amida Care Live Life Advantage on the first day of the month after you sign the enrollment application. You will receive a confirmation of enrollment letter with your effective date.
